First Impressions & Appearance
I like your theme. It's simple, but it still looks nice. The color scheme is attractive, and you've got plenty of sports icons that ensure that people know they're looking at a sports forum. I wouldn't make any changes to your theme. Your theme has a good balance of simplicity and attractiveness.
Your site also looks pretty active at a glance. You don't have any empty forums, and you've got plenty of posts and members. I doubt that any prospective member would shun your forum due to lack of activity.
I don't usually say this, but my first impression of your forum was about as close to perfect as it gets.

Site Layout
Your forum's index could be a bit more concise, but I honestly don't see any forums you could merge or delete to save room. You seem to be using the most minimalistic layout possible for your forum. One thing that doesn't make sense to me about your layout is the fact that "Basketball, "Football/Soccer," and "Hockey" aren't separated (they're together without any borders) when those sports don't really have much in common with each other. You may consider fixing that. Other than that, though, your layout looks really good.

Statistics and Activity
According to my calculations, your forum currently averages just under one hundred posts per day, and you have 144 posts for each of your 346 members. These statistics really aren't bad, but there is still room for improvement. You don't seem to have a lot of new members joining each day. You should probably step up your advertisement. You have a very active community, so advertising on other forums in your signature or in promotion sections may be a good course of action. Be sure that the forums are sports-related, though, since your target audience will be on those types of forums (of course). You should also send out a mass e-mail if you haven't already. Since your forum has been up for a long time, it's possible that many users have forgotten about it. Send out a little e-mail just to say that you're still going and would appreciate seeing some older members. Like I said, though, your statistics aren't too bad. You are getting many posts every day, but I think that you could do better.

Staff Moderation
If I can count, you currently have six staff members. For your activity level, it seems that you may be a
tad overstaffed. However, all of your staff members are at a good activity level. That's good; if your staff members post often, then there will be more content for users to discuss. However, if you lose a staff member, don't be afraid to leave his/her position vacant. Another problem I noticed is that not all of your staff members are actively welcoming new members. Encourage them to do so; members are more likely to stay if they feel welcome.

Spelling/Grammar & Professionalism
You have done a fantastic job of writing your descriptions. Each description is at least one or two complete sentences, and your descriptions lack any serious grammatical errors. There is one "big" mistake I spotted, though. In your "Baseball" description, "Well, that doesn't matter, let's just discuss America's past time here!" should read, "Well, that doesn't matter
; let's just discuss America's past time here!" Other than that, though, I can't spot any errors that most people would find. Good work. The descriptions reflect the amount of effort you've put into this forum.
