How to add a contact form in IPB forum?

I am using IPB for my forum, and now I would like to add a contact form somewhere, like 'contact us' icon, and when someone clicks it, a form will show up. Could anyone tell how to do it? Thanks!

Richy, there is a modification for Contact on the IPB Marketplace.
