None of my own staff, no. I ensure my circle of trustees are quite trustworthy.
However, I did have an instance of this occur at a promotion forum I staffed as the Community Team Leader a few years back. The owner went on vacation and left a certain temporary administrator in charge. A fair portion of my fellow staff members did not take well to him and banned him from the forum. The temporary administrator also did not respond well, demoting all of the staff members (including myself) and changing their passwords.

I've never had to deal with anyone doing it to me.
I did administrate a forum where one super administrator was angry at the owner and started messing up the forums. Another super administrator kept reverting everything that they did, however they didn't have the ability to remove the other super administrator due to the super admin status being a configuration file flag.

Haha what if they were? Only thing I think people find regularly is that staff may not follow your guidelines to literally. I think this happens when people volunteer efforts. Some sort of loosely interpret what you say with a bias to their own convenience.
