Step 1: Open Photoshop and create a new document with a transparent background with the size: 1440 x 40 (width x height).
Step 2: Open the stripe patter you generated on http://www.stripgenerator.com. Click Edit -> Define Pattern and save it as whatever you want to call it. Now close this file and go back to the new one you just created.
Step 3: Take the Fill Bucket and at the top where it says "Foreground Color" click the down arrow and click Pattern. Now in the little box next to it click the arrow and select the pattern you just defined. Paste this into the canvas.
Step 4: Create a new layer and measure the size of the content area. I'm not sure on the exacts here, but if you install Pixel Ruler (search google) you can get fairly exact. Click the Marquee button on the tools menu (or Click M on the keyboard) and at the top set it to Fixed Size. Enter the height (40) and the width you found for the content area. Add 20 to the width (That's 10 for each side; you can add more or less based on preference, but remember that you divide that in half for how much will stick out on either side).
Step 5: Use the marquee tool to select in the middle of the canvas. Fill it with the color you want for the side vertical "poles", and adjust the opacity to how you want it. You can add a stroke (border) to it by double clicking the layer in the Layers Pallet and clicking Stroke at the bottom.
Step 6: Create a new layer. Go back to the marquee tool and now decrease the width by the amount you increased it above. Use the marquee to select in the middle of the previous layer, and fill it with White.
You're done. You can style it up, such as adding a Drop Shadow on the top white layer if you want. Just try and be original and don't do it too much like here or it will all look the same. 🙂
