Only really two, I mostly leave the bans to other people. One from FP who posted a lot of spam and thought it would be cute and benefit their site. And a clown who acts like he's the center of the universe.
As for porn, I don't care as much as I might have used to, although I know some admins who will go berserk at the sight. My problem is that younger people will see it and that causes all sorts of problems and it's not the sort of thing someone'd want seen at work.
If there's a R18 section, then that's the place to do it, but even setting one up is too much hassle 90% of the time, although I know admins who have.
