Creating a Facebook Fan Page

Hi.

I want to create a facebook fan page, but when I go to this link:
http://www.facebook.com/pages/create.php

I get multiple options as to what I need to click next:
http://imageshack.us/photo/my-images/90/facebookrx.jpg/

I want to create a Fan Page that's in relation to my blog about mental illness.

What would I do at this point so I can successfully make a Facebook Fan Page?

I would say Cause or maybe Organization, but probably more of a cause.

Causes would be a better choice since pages are more for celebrities, company, websites, and blogs.
