I agree with the examples you mentioned above.
Personally, as an admin, I think it reflects poorly on a site to allow people to have offensive usernames. It sends to potential members that this is the kind of attitude and behaviour they can expect from members here. Which can create a rather hostile and unwelcoming atmosphere on that site. (And that can turn people off from joining. And for most sites admins already have enough trouble getting members to join and be active to begin with, they don't need something like this to factor in.)
And seeing a user register with an offensive username only makes me think they're really not going to fit in on my site because they don't have the maturity or ability to respect others that they would need to be part.
So for really offensive usernames, I'd change it first and then let them know I'd done so. This way they don't get to weasel around and try debating with me while their existing name is used/showing up and potentially is offending people or turning away potential members. The name gets changed and hopefully they take away from it that offensive names aren't permitted and I won't tolerate that sort of misbehaviour.
I mean if someone joins with an offensive name, what are the chances they're going to be a good contributing member?
In my experience, most of the time only trolls/problem members join with offensive names. Legitimate members will join with non-offensive names because they expect to be around longer and become part of the community.
Besides, if they are a good albeit clueless member then they'll be okay with having their name changed when they read the email explaining it. (Because they probably didn't even think/realise that the name they picked was offensive in some way.)
While if they're a problem member (which is 99% of the time), they're going to respond negatively and get removed instantly anyway.

As for any ideas of free speech? On your site you can do what you want... But on my site free speech is limited to be only free so long as it's not offensive to others. (Or essentially, users can state whatever they want, but if they make offensive comments then they have to deal with the consequences of their comments.) So, for example, using racial slurs for a name is not justifiable by any regard. (You're still free to make those comments, but I'm also free to take action like deleting those comments or changing offensive usernames.) I do not want that sort of behaviour on my site at all. I do not want members or potential guests to be uncomfortable/insulted or turned away because one person's username was offensive.
And if one person's idea of fun is offending people? I'd rather they weren't on my site. They're probably just a troll anyway and my site will be better off without them around. Because like I pointed out in my quote, if they're joining with an offensive username, they probably aren't there to be a good/contributing member of the site to begin with.
Censorship is not all bad all the time, it has it's place too. People have a right to be free from sexist, homophobic and racial slurs. And (on my site) if it means taking away one person's right to free speech to protect others from having to hear grossly offensive comments, I will do that. One person's right to say whatever they want will always be outweighed by other's rights not to hear hate speech.
