A standard sized favicon ranges from 16*16 to 20*20, depending on browser. I have never heard of a 48*48 favicon or a 12*12 favicon (for browsers) before 😕

16*16 or 20*20. I recommend 16*16, because if you make a 20*20, and the browser only supports a 16*16, it will appear crammed (even though a 16*16 space would be crammed 😉)

Actually, you can embed a favicon to be multiple sizes depending on the situation;
It is typically just a 16x16 icon however if made correctly you should make it 48x48, 50x50, or even 100x100 (at least 48x48 - anything square & bigger is fine) and then have it resized automatically so that when it's in a browser it's 16x16, when someone bookmarks the website as a desktop icon it should automatically show as 32x32 or 48x48 depending on their settings.

So, such as my favicon on my website: http://www.gfxdrop.com
it shows up as a 16x16 favicon (12x12 to some settings) on the browser, however if you create a desktop shortcut it automatically makes the 48x48 desktop icon with the same favicon (with the correct image dimensions so it's not a distorted stretch of a 16x16 favicon):

Anywho, enough of my rant of correcting the favicon information;

I'll make one of these for you tonight.

If you create a 50*50 favicon, and it gets resized by the browser, and it is not a .ai (vector) image, then it will appear blurry as it has not been created to be resized, thus why icon sites have different sizes. Resizing will cause 4 pixels to try and fit into 1 thus making it blurry. So, that's why 16*16 is recommended, as you do not need massive resizing.
 
when the actual favicon.ico is created from the image it resizes the image then. it doesn't get resized by the browser every time it's loaded and its not blurry at all.... look at my sites favicon, that was made from a 50x50 image into a favicon, i didnt resize it at all and it's most certainly not blurry and it has a 48x48 desktop icon embedded into it
