If you have a website or a blog and you want to make it easy for the people who visit your site to leave feedback, there is a website that can be beneficial for this purpose.

It's called Feedback Forms located @ http://www.feedbackforms.com/

What you do is create an account and answer a few questions. You will then be able to create the feedback form based on your needs.

Once you have the feedback form ready, you then paste a bit of code onto your site or blog, and it will generate a "leave feedback" balloon on the lower right corner of your sites/blog's pages.

This will allow your visitors to click the link, fill out a few questions that you specified when you created the form and their feedback is stored. When visiting your account, you will see the 10 most recent comments made by your visitors.

The free version only allows you to see the latest 10 comments, and you can only have the code on one site. For $9.95/mo., you can have it view the latest 100 comments and you will be allowed to use it on more than one site.

Nice looking service Andy, have you tried it on your own site? I'd be interested in seeing examples of some sites that are using it.
 
I do have this service on my blog. You will notice that if you visit my blog link, you will find that the feedback link is in the lower right corner of the window/page.
