Sometimes it doesn't work. The alerts and/or email notification doesn't. I've been noticing that lately. So do you all notice it? Is it the forum software's fault or is it the forum owner's negligence (not doing maintenance)?

Which forum software are you talking about?
I've seen that on ProBards, MyBB, XenForo, probably more.
Email is a tricky thing because email providers can be very aggressive in order to cut down on spam. All it takes is getting on the wrong side of an algorithm, this is fundamentally a problem which is outside of the forum software developers' and webmasters' control.
If it's just an alert, then it's probably a bug, it's probably the forum software developers' fault, although nothing is perfect, it really depends on how frequently it happens.
