First Impressions & Appearance - (Out of 20)
First thing I noticed was the broken image before "The Hockey Nation" text. t looks unprofessional to find broken images on forums, so I'd get that fixed. The menu text (register, help, etc) as well as the text on the bottom of the forum (contact us, etc) is hard to read. I pretty much need to highlight it.
I don't understand why you have
[suspended] listed in the member legend. I'm assuming suspended means banned, so it makes no sense to have the group listed if you won't see them online.

Site Layout - (Out of 20)
The first few things I see when opening the forum is the sports bar at the top, and the Chatbox. When I'm viewing a forum, a chatbox is not what I wish to see, especially not the first thing I see. I would make the chatbox member-view only, or move it inside a board and make that board member only.
Back to the sports bar feed, I clicked the 'X' thinking I'd close the bar, I ended up closing the tab for the forum. I'm not sure if I'm confused on that, or if there's a little mix around with that. I'm not a fan of the overview where it's at. I know you'd like the stats to be visible, but I personally wouldn't want to see that if I'm going to be considering joining your forum.
The tabbed layout of your categories has me impressed. These sort of tabbed categories are very smooth and slick. I noticed a few sections on your forum have no posts at all. Its' best to add posts in empty sections, even if it's just a sticky topic.

Statistics and Activity - (Out of 20)
Well, first of all,
Our members have made a total of 7 800 posts in 449 threads
Where is the comma between the 7 and 8? That will really put people off. With only 36 members, 7,800 posts is pretty nice. Most of your members seem active, which is pretty obvious with the post counts.
Looking through the members list, there are still no commas between the post counts. If this can be changed, I would fix it immediately.

Staff Moderation - (Out of 20)
In total, there are 6 staff members. I'm not sure what role the Managers and Coaches have, but I don't think that many staff is needed. If anything, I'd remove Managers and Coaches from the staff list, unless they have some important role.
On the bright side, all of your staff are active post wise, as well as when they login. On a final note, I wouldn't advise adding more staff.

Spelling/Grammar & Professionalism - (Out of 20)
I can't seem to find any problems with the board's descriptions. You seem to be very professional with your members and with being an admin. This goes along with what I said earlier, but I'd get those broken images fixed. Otherwise, it can look really unprofessional.
