How and where do I add this code to the CSS in phpbb3?

Open styles/yourtheme/theme/stylesheet.css and add to the bottom:

Otherwise, just add your custom CSS to the bottom of styles/yourtheme/theme/colours.css.
 
I added the code! How do I create the BBcode? The tutorial I got this from is for Mybb so I am confused.

Sure. For instance, if you want the warning box to appear, just replace info with warning and so on.
