Methods of Blocking Spam

What are some methods or plugins you would use to block spam on your website or forum?

I used to always use this plug in on my MyBB forums: http://mods.mybb.com/view/stopforumspam-com-for-mybb. It worked very well for me, for the most part.

I sometimes use security questions and currently I am using Sortables Captcha. I thought they helped but I never seen a forum without spam. These are good with stopping spam bots but they do not stop human spammers which is a shame.

I use reCaptcha, Stop Forum Spam, and email verification. I also have a plugin set up to identify and report any spam that comes through. The system works pretty well.
