Not technically just a music video, but I guess Broken, for the EP of the same title by Nine Inch Nails could count as its presented as a snuff film, never officially released though.

I find the end of this song, at around 2:53, to be the most disturbing music video for any mainstream band, though it can't compete with a lot of lesser bands..
