Social icon its not showing because you have uploaded your forum in folder /forum
you have to changed icons directory to /forum/images/....
Loginbox is showing when logged in because you have to changed fid number to 1
2=registed members
1=guests
About error 500 I dont know,
I have tested that theme many times and its working fine.
It may because a bad plugin
