First Impressions & Appearance - (Out of 20)
I really like this theme, but it's really loud! I think that a lot of people would get eye pain from looking at your theme. You should tone it down a bit so that it's not so bright in spots. First of all, the banner is
way too bright. If there's any way you can burn it or whatever to make it less bright, then you should do that. It's a really nice banner, but it's simply way too bright for your otherwise dark theme. The cell pics are also quite bright for your otherwise dark theme. The cell pics, if you don't know, are the blue and white that are the background for the category headings (e.g. "Introductions," "Out of Character"). Again, if there's any way you can burn those to make them less bright, then you should definitely do that. I think that your theme looks great, but it would benefit from being a little less bright in spots.
Your forum looks quite good from an activity standpoint. You don't have any empty forums, and you have plenty of posts and members. I doubt that anyone would shun your forum based on activity (at a glance anyway).
Overall, my first impression of your forum was pretty good.

Site Layout - (Out of 20)
Your forum does not have many forums on its index, but each category has only one forum. That makes your index much longer than it should be. I think that you should reorganize your forums into two or three categories. One idea I have is to divide your forums into three categories: "Naruto RPG Headquarters" (news forums, introductions, and off-topic discussion), "Out of Character" (guides and other forums that don't actually involve roleplaying but are vital for players to use in order to roleplay effectively), and "Roleplay" (forums where players can actually roleplay). I'm sure you'll want to give these categories Naruto-related names, but there's your general idea. If you used categories effectively, your layout would be great. Right now, it's pretty disorganized.

Statistics and Activity - (Out of 20)
According to my calculations, your forum currently receives about forty-five posts per day, and you have 121 posts for each of your 137 members. First of all, you have an amazing post to member ratio. This shows that your members have a very high level of dedication. You also don't have a bad post per day average for a roleplaying forum. However, I think that you can do even better. You should definitely be sure that you're promoting your forums on anime-related forums as well as roleplaying forums. Post around on other forums with subjects similar to yours with your link in your signature. If you make constructive posts, then others may be inclined to check out your forum. Since your forum has been up for almost a year, you should send out a little e-mail update to all your members just to let everyone know that the forum is still around. In a year, it's possible that some people have forgotten about your forum. You can also find many tips for search engine optimization and the like here on Forum Promotion.

Staff Moderation - (Out of 20)
It appears that you have two administrators and two moderators. For your activity level, you are sufficiently staffed. Both of your administrators are extremely active on the forum, but your moderators are less active. You may want to encourage your moderators to post more. If your staff members post more, then your users may have more to discuss, causing them to post more. Also, not all of your staff members are welcoming users to the forum. Since you have four staff members, every introduction topic should have at least four replies. Encourage your staff members to welcome new users. If your new users feel welcome, they'll be more likely to stay.

Spelling/Grammar & Professionalism - (Out of 20)
All of your descriptions on your main page are great. They're all fantastically long and detailed, and I really don't see many grammatical errors at all. However, once you click into the forums and view the sub-forums, the quality of the descriptions declines. I don't think that the sub-forums matter as much as the forums on the main page, but you may still want to see what you can do about the descriptions of the sub-forums.
