Need help finding a MyBB Mod

What is the best mod to use to add forms automatically when you go to create a new thread in a specific forum? I used to have it set up on Admin Village, but I moved hosts and some how lost it in the process. If it's not a mod, just tell me how to do it.

This was the mod I used. It was originally in Spanish though it had an English translation (which, if I recall right, is included).

It's a fairly simple mod, I don't know if it's the best... But it does work.
