Something weird is going on. I quoted both @Deathstarr and @meetdilip but only one got inserted when I pressed okay.

I have the usergroup for Registered as one of the selected, but went ahead and checked a few more usergroups that you two are both set in for your accounts. Please let me know if it works now?
 
Something is going on with the form. Staff were able to fill it out...

Please don't try anything until I can figure it out or get @Ozzy47 to take a look at this. Sorry for the delay. I'm not sure what went wrong.
 
I figured it out...only Staff has permissions to utilize the forms and that includes viewing them. Let me see if @Ozzy47 wants me to change that or use a different method for this survey. Sorry about this, everyone.
 
I tried to fill it out but it returned a generic error message so I couldn’t submit it.

Do you have my response? I tried to reply to another thread and got the same generic error. So perhaps it still went through?
