Some forums have User Notes where staff can notate things that aren't listed in warnings about members such as sending a PM for flaming, or putting them on a restriction for their account. It helps for new staff to know what's going on with that specific member.

Should members be able to see their own user notes?

Personally I don't see why not. If it is used by staff to log their punishments then the user already knows they were punished and for what. So I see no reason why they shouldn't be able to see their own notes so long as it is only viewable by staff and the user and no outside user.

You have staff notes that most staff can see and read the fine details about that member, then you have the warning sent messages that the account holder sees and reads 🙂

This makes sense. Having 2 different notes. 1 that only staff see and 1 that staff and the user can see.

A forum where I used to be active on showed when and why you got the warning. The staff would send you a message and politely tell you why you got it. Like for instance. I got a warning for posting spoilers on a movie that I found on Youtube. I didn't know it was a spoiler, but it didn't stop the owner from slapping me with a warning anyway. He told me he needed to make an example of me so other people wouldn't do it and then he told me that I was pretty good stand up guy. I guess I can respect that.
