You want to hear about Extreme spamming... Well I left this site to sit and I actually forgot it was there...

I had a news feed set up so that relates to a few posts but check these stats.... I told no one about this site as I was only just in the making when forgot about it after batms.com got deleted!

Anyway:
Threads: 2,622, Posts: 4,088, Members: 343, Active Members: 270 ~eeeek~

That would stop 99%+ of them spammers signing up.

Actually the vBulletin Image verification is next to useless for preventing spam registrations, best way to reduce it would be to install this little Add-on -> http://www.vbulletin.org/forum/showthread.php?t=124828

The question and answer system is the add-on I gave above. I had to install it on the PixelRaider forums, however did not 100% stop registrations.

The Q&A comes as standard in vb 3.7... That mod you posted is for 3.6.

You mean the NoSpam! verification?

I didn't actually check the link, but if you're referring to NoSpam! then 3.7.x does not come with it.
