Let's talk about some things that might make a very active forum go dormant.

Some common reasons might include:

Inactivity by forum management/owner
Popular/active members leave and are not replaced
Active promotion of the forum stops
Spammers over run the forum making it hard to find real posts

Another big reason that a forum might go dormant is when the forum owner stops caring about welfare of forum and just stops trying. Most times, forum members will take their cues from the forum admin or owner. Another mistake is relying too heavily on just a few active posters or friends to keep forum going and when those people leave, the forum is suddenly dormant.

If your forum ever went dormant, what are some factors that were involved?

I think that you have pretty much listed everything. 😉

I think the biggest reason is:
Forum moderators or admins are arrogant and they annoy other members by deleting their posts unnecessarily.

A poor staff team and/or a bad forum atmosphere, which both likely go hand in hand. No one will stay at a forum that isn't nice to be on, regardless of what nifty things it might offer. It'd be like a restaurant offering an amazing dish, but the actual restaurant doesn't offer a nice experience to make you notice or want to stay for that dish.

Lack of new content. If you see old posts and such, that's very likely to put you off. There should be new content, to get others to join in to contribute to the activity. 😛

Site going down for about a month because your hosting company needed changing and your next host didn't work properly.

I know, because that's what hit me recently.
