What do you ask for in your application?

So if you are one to open applications for staff members, what do you usually ask for in your application? Do you ask for their age or past experience, or even their gender? What is on your staff application, and what is required on your staff application?


For my applications, usually I just have name, age, and past experiences. Sometimes I will throw in a few "questions" like "Why should I hire you?" and things of that nature, however that's about it. I am also one for not requiring anything on the staff application, members can fill out what they want and leave the rest blank. I don't feel right requiring anything from a member, and if they choose to leave out certain fields, then that will affect my judgement on hiring them. So it's "required" but not exactly "required" if that makes sense. 😛

What's on your staff applications?
 
I usually ask for a few personal details, time zones, age, etc. I also asked about past experience and personal qualities as well. I tend to sometimes throw in a few job specific questions.
 
  1. Age
  2. Gender
  3. Location / Hours of being online / Timezone
  4. Primary language / secondary languages
  5. Experience as a forum admin, mod, etc
  6. Explanation of the purpose of my website
  7. How they can help my website, specifically

I ask for a variety of things depending on whether or not I know the person well or not. I usually know my staff pretty well, so I don't have to ask too many questions because I already know the answers. For newer projects or staff candidates I don't know too well, I ask at least the questions I listed above as well as some others.
 
I ask them to send me an application, that's it. I get a pretty accurate impression of the person applying by seeing what they value in an application, and prioritise.
 
The basics:
Name
Email/Skype/etc.
Age

And then other information like:
Why they want the position.
Why we should hire them.

Before applying they need to have a certain amount of posts, determined by staff.
 
I usually ask basic stuff but most importantly, why do they want to become a staff member.
