When Did You First See The Windows "Blue Screen of Death"?

Used to see this almost daily (or hourly) when using my laptop when Windows 10 came out. Seems like even now it still acted up, and the reason is due to a Synaptics driver fault in where Synaptics installs an updated driver and Windows automatically will reinstall an older version.

I've tried everything from new, clean install of 10, installing drivers as Windows 7 permissions, and tons. Only thing that has helped was a clean install of something non-Windows related.
 
I first saw it while I was playing Cookie Clicker back in late 2013; some friends and I were really into it at one point. It just came up randomly, and it would come up whenever I opened the game and even after that. Took my computer to a few computer repair shops and I was really concerned about it because it was in the middle of a semester and I needed my laptop to do school work, and I didn't want to get a new computer because I'm not a fan of Windows 8 (or 10 for that matter, but 8 was the big thing at the time).

Turned out it was something that needed a driver update. Did that and the problem went away.
 
Never had a Blue Screen yet (touch wood), but I did have a 'fake' Blue Screen caused by Malware on Windows 95 😛

Also I had to take it to a repair shop, at that time I knew nothing about Security.
