Why emails go to spam/junk?

What is the website name, URL, and content of the email? It's going to depend on the person's individual spam filter settings, however, your website's name or URL could be causing part of the problem.
 
Sometimes when emails are being sent out on a rapid basis, like when someone has a box ticked to send them emails whenever something happens on a forum, the emails end up being automatically labeled as spam, because of allot of emails being sent over a short periond of time.
 
I don't know. I know that if a particular server is known to send spam out, any emails coming from that server can potentially be labeled as spam.
 
It also depends on the content of the email. Some registration emails contains too much of advertising and subscription that lead them to land in your spam or junk folders.
 
If the e-mail provider is using an anti-spam database (like Spamhaus, etc), the manager of the anti-spam database could be blocking the IP address from the last user of the IP, or the current user. You could check that.
