He didn't go to PL1. That was the issue there. I've never been hacked either, for the years that I've been on SMF, IPB, MyBB, and vBulletin (though one MyBB forum I used to own got hacked because the new owner failed to upgrade between versions and was three or four versions behind the latest release).
It was this vulnerability found in 3.8.6 that screwed him over.
